About the SLC “Maiale” – Manned Torpedo + 2x Frogmen:

Nicknamed the "pig" because it was difficult to steer, the Italian Maiale was the first truly practical human torpedo. The official Italian name for most of the craft that were manufactured was Siluro a Lenta Corsa (SLC or "Slow-running torpedo"). Used as a secret naval weapon in World War II, the Maiale was piloted by two crewmen equipped with an oxygen rebreather apparatus. These two soldiers would steer the craft to an enemy ship, which often took as long as two hours at a depth of about 49 feet. From there the crew would detach a warhead to be used as a limpet mine. Then, if not detected, the crewmen could ride the sub away to safety.
